Service Agreement

 This is NOT a contract!

Mantis’ Service Agreement will take effect at the beginning of the first service and last until the services are canceled by the customer or Mantis Pest Control. There are no fees or contracts involved with Mantis Pest Control’s services. However, if the customer cancels all of their services the day of or when the technician arrives at their property, the customer will be charged fifty percent of the total owed for that specific scheduled service.

This service agreement is between Mantis Pest Control and the customer. No other party or representation shall be involved. All terms and conditions of this agreement will remain in effect until the agreement is terminated by either party.

Satisfaction Guarantee

If the customer is not satisfied with the results of any service at any time, Mantis Pest Control will return within that 30 day window to complete an additional service at no cost to the customer. While we’re there, we’ll conduct an audit so that we can assure both parties that the problem(s) will be resolved. If, for any reason, both parties are unsatisfied, the service agreement can be canceled at no additional fee by either party for any reason at all.


Scheduling & Customer Responsibilities

Customers will receive pest control services depending on the service route for the specific day and the customer(s) will receive notifications by phone, text and/or email. If the customer cannot be there on the specific day, the technician(s) will treat the outside thoroughly and, if requested, we can return to complete the inside at no additional fee.

At Mantis, we need the customer’s full responsibility to eliminate conducive conditions regarding pest presence and/or infestations. All recommendations should be completed to the best of the customer’s ability as well as any repairs needed to prevent current or future pest infestations. These recommendations and observations may be verbal or written on work orders or other forms provided by Mantis Pest Control and its employees. Mantis Pest Control is not liable for any outcomes by other parties in any way, shape or form.



Service Limitations

The effectiveness of pest control treatments are limited by site, structural, and environmental conditions which are beyond the control of Mantis Pest Control. Such conditions include, but are not limited to, inaccessible structural cavities, leaking roofs, faulty grade levels, inadequate clearance, earth to wood contact, structure- vegetation contact, cellulose debris, insufficient ventilation, faulty gutters or down-spouts, and other conditions conducive to re-infestation or recurring infestation. Unless Mantis has specifically included the repair or correction of such condition(s) on the face of this service agreement, the customer accepts sole responsibility for such repair, correction, or mitigation, and holds Mantis Pest Control harmless from any liability for same, including any responsibility for re-treatment. Mantis Pest Control is not responsible for insect or rodent damage to products or contents at the premises. This service agreement does not provide for control of pests not specified. Mantis Pest Control reserves the right to refuse requested service without incurring any liability for any reason especially if the job would be endangering its own employees or the customer(s). The customer agrees to hold Mantis Pest Control harmless from any and all liability for damage not caused by Mantis’ negligence, and from any and all liability for structural damage regardless of cause. Mantis Pest Control disclaims any liability for special, incidental or consequential damages.

The warranties stated in this service agreement are given in lieu of any other warranty, expressed or implied, including any warranty of merchantability or fitness for a particular purpose. Mantis Pest Control will incur no liability should it be prevented from fulfilling its responsibilities per this agreement by reasons of an act of God, fire, flood, storms, explosions, acts of war, whether declared or undeclared, seizure, riots, civil commotion, strikes, or other labor disputes, shortages of fuel and/or materials, quarantine restrictions, or any other circumstances or causes beyond the control of Mantis Pest Control. Should structural changes or additions be made to the job-site which would add to or delete from the area treated at each service, the periodic service charge will be reviewed and adjusted accordingly by Mantis Pest Control.


Payment is due upon completion of the service. This is null only if the customer has paid for the service(s) ahead of time or some other arrangement has been made. Payment(s) not made by the 30 day mark will be sent to collections for maintaining an unpaid balance. Mantis Pest Control can terminate or suspend this agreement and the customer can be held responsible for the costs of fees regarding administrative employees, attorneys, and collections.

Rodent Control


Any bait stations placed around the residential/commercial property will be removed upon the cancellation of the service(s). Bait stations are placed to help customers prevent future rodents from entering their property. These bait stations are exclusive to customers who are on recurring services with Mantis Pest Control.
